I think an awful lot of managers gauge what they believe a player should be rated against another player they deem relative or comparable.
Not many managers use the suggested ratings guide as an absolute - note the not many. Just because a player isn't playing international matches doesn't mean he's not worthy of a higher rating. Similarly just because he might be doesn't mean he warrants one.
I think most players playing regular top-flight football (yes, I understand both "regular" and "top-flight" are subjective terms) warrant a rating of 79ov. or above, and then it's just your own personal choice of who's better, edging up the ratings gauge.
In theory a common consensus should then be able to dictate who's at what rating, and on and on.
It's an imperfect system, clearly, but it's what we have.

I understand about certain players being better than their rating even if they aren't playing International games on a regular basis. Spain and Germany have a glut of players so there are players who won't get in the team. But there aren't many with that amount of talent.
